Built-in SOAP Note
Your normals, your specialty
The SOAP Note’s Objective section is yours to customize. Pick your specialty and PawfectNotes uses that specialty’s physical exam; then edit the default normal findings for each vital and body system so normals read exactly the way you write them.
- Assume normal when not mentioned: findings you don’t mention are written with your normal text instead of being left blank.
- Use default normal language for normals you mention: say “ears look fine” and the note reads “Ears: Clean with no discharge or erythema AU.”
- Reset Normals any time to go back to the PawfectNotes defaults. Notes already written are kept.
- Need to change Subjective, Assessment or Plan? Duplicate to Edit copies this specialty’s physical exam and your normal findings into your own template.

Your templates
Build custom templates in minutes
Recreate the template your clinic already uses, or design a new one for rechecks, dental cases or lameness exams. Every template you build works for every note you record.
Open Templates and choose New template
Under Your templates, choose + New template, or duplicate any template to start from a copy.
Name it and choose who can use it
Give it a name, then pick Only me or Practice to share it with everyone in your practice.
Add sections and fields
Add a section, then add text fields, vitals or a physical exam inside it. Reorder with the arrows and remove with ✕.
Pick how each field is written
Bullet points, a paragraph or one sentence, per field. A template can hold up to 150 headers and items.
Save and record
When it shows ✓ Ready, save. Your template is ready for your next note, and you can set it as your default with the ☆.
Share with your practice
Decide who can use each template when you save it. Change it later whenever you like.
Only me
Only you can use it. Perfect for your own shorthand, rechecks and go-to formats.
Practice
Everyone in your practice can use it, so the whole team writes notes in the same format. Only its creator or a practice manager can change it.
Recording for another vet
When a teammate records for a veterinarian, the note starts from that vet’s default template, specialty and language.

Can I customize the built-in SOAP Note template?
Yes, its Objective section. Choose your specialty and set your own normal findings for vitals and the physical exam. Subjective, Assessment and Plan are fixed in the built-in template; to change them, duplicate it and edit your copy.
How do I build my own template?
Open Templates, choose New template, give it a name, then add sections and fields. Each text field can be written as bullet points, a paragraph or one sentence, and any section can include vitals and a physical exam. Save it and it is ready for your next note.
Can I share a template with my clinic?
Yes. Set who can use it to Practice and everyone in your practice can use it, or keep it to Only me. Only the person who created a template, or a practice manager, can change it.
What does “Assume normal when not mentioned” do?
With it on, physical exam findings you do not mention are written with your normal finding text, for example “Ears: Clean with no discharge or erythema AU.” With it off, they are left blank.
How large can a custom template be?
A template can have up to 150 headers and items, and its name can be up to 100 characters.
Do custom templates work in my language and with my PIMS?
