Set Your Objective Normals

In the built-in SOAP Note, the Objective section is yours to customize: set the default normal findings for each vital and body system so normals read exactly the way you write them.

Step 1: Open the SOAP Note template

  • 1Open Templates in the app.
  • 2Under Built-in, choose SOAP Note.

Step 2: Check your specialty

  • 1The Objective section shows Specialty: …. Choose it to pick a different specialty.
  • 2The physical exam follows the specialty, and your normal findings are kept for each specialty. See the Specialties guide.

Step 3: Edit your default normal findings

  • 1Vitals: type the default normal for each vital, such as temperature, heart rate or respiratory rate.
  • 2Physical exam: type the normal finding for each body system, for example “Ears: Clean with no discharge or erythema AU.”
  • 3Once you customize them, the SOAP Note card in Templates shows Customized with a link to its specialty.

Step 4: Choose how normals are written

  • 1Assume normal when not mentioned: findings you don’t mention are written with your normal text instead of being left blank.
    Ears never mentioned
On: “Ears: Clean with no discharge or erythema AU.” · Off: left blank
  • 2Use default normal language for normals you mention: normals you mention are written in your default wording.
    You say “ears look fine”
On: “Ears: Clean with no discharge or erythema AU.” · Off: “Ears: Ears look fine.”

Step 5: Reset Normals any time

Choose Reset Normals to go back to the PawfectNotes defaults. Notes already written are kept.

Change Subjective, Assessment or Plan

In the built-in SOAP Note, Subjective, Assessment and Plan are locked (“Not editable — duplicate the template to change it”). Choose Duplicate to Edit: it copies the template, including this specialty’s physical exam and your normal findings, into your own template that you can change completely.

Next: Build a custom template.
